CareConnections has positioned themselves as a leader in assisting clinicians with evaluating the status of their patients from the moment they're seen to the second they're discharged. The data we collect includes:

  • Rating of change during subsequent visits

After this data is collected, management can put together reports looking at data across multiple orientations, including diagnosis, risk-adjusted groups, site of service, and individual care provider.
